We undertake assessments to understand individual needs, eligibility, and appropriate supports, ensuring services are tailored and responsive.
Flexible Service Response
Outreach
Outreach support for individuals who may be isolated or require additional assistance to engage with services.
Social Support
Assistance to build social connections, reduce isolation, and support participation in the community.
Community Care
In-home support to assist with daily living tasks, helping people remain safely and comfortably in their own home.
Property Maintenance
Support with basic property maintenance to ensure a safe and accessible living environment.
Who Can Access HACC PYP?
HACC PYP supports people who:
• Are under 65 years of age (or under 50 for Aboriginal and Torres Strait Islander people)
• Have a disability, chronic illness, mental health condition, or other circumstances impacting their ability to live independently
• Require support to undertake everyday activities and remain living safely at home
• May need one-off, short-term or ongoing assistance
